RESUMO

Nuestro objetivo ha sido comparar la eficacia de 100 UI/día de calcitonina más un gramo de calcio, con 400 mg de etidronato más un gramo de calcio y 800 UI de vitamina D, y con sesiones de magnetoterapia en el tratamiento de mujeres con osteoporosis postmenopáusica, a lo largo de dos años.Se estudiaron 60 mujeres postmenopáusicas (menopausia mayor de cinco años) que presentaban, bien aplastamiento vertebral (independientemente de la densidad mineral ósea -DMO- que tuviesen) o bien una DMO 0,82 gHA/cm2 sin aplastamiento vertebral. No se admitieron en el estudio las pacientes con aplastamiento vertebral comprendido entre L2 y L4, peso inferior al 20 por ciento por debajo del peso ideal o superior al 50 por ciento, pacientes con tratamiento que interfiera en el metabolismo óseo, así como casos de osteoporosis secundaria. Las participantes sin criterios de exclusión fueron distribuidas a medida que acudieron a consulta (reclutamiento) en tres grupos de tratamiento, constituido cada uno por 20 pacientes. El seguimiento consistió en revisiones cada seis meses, con control clínico y realización de densitometría ósea (columna y fémur), así como analítica con hemograma, bioquímica general y parámetros de remodelado óseo. Cada 12 meses, se practicó un control radiológico, con radiografía de columna dorso-lumbar. En el análisis estadístico se realizó la comparación de variables cuantitativas, utilizando el test estadístico adecuado según el número de grupos que establece una variable cualitativa (t-Student o ANOVA para variables de distribución normal).Al cabo de los dos años de tratamiento, los pacientes que recibieron etidronato más calcio y vitamina D, habían aumentado su masa ósea en un 4,59 por ciento a nivel de columna y en un 2,83 por ciento a nivel de cuello de fémur. Este aumento fue superior al conseguido con los otros dos tratamientos. La diferencia al año de tratamiento fue estadísticamente significativa respecto al grupo de calcitonina más calcio y al de magnetoterapia; aunque a los 24 meses, esa diferencia sólo fue estadísticamente significativa respecto al grupo tratado con calcitonina. Los resultados obtenidos por la magnetoterapia han sido un efecto de mantenimiento de la masa ósea, tanto en columna como en fémur (AU)

The defect of the sodium pump of the blood cell membrane in patients with essential blood hypertension, can induce changes in the metabolic use of ATP, especially at glucolytic pathway level. We studied the blood levels of ATP (32.51 +/- 10.80 mg/100 ml), 2,3-DPG (0.82 +/- 0.47 mumol/ml), piruvate (0.36 +/- 0.16 mg/100 ml) and lactate (18.88 +/- 4.53 mg/dl), of a group of 50 patients with essential blood hypertension and of 50 healthy persons in the control group (17.09 +/- 6.37 mg/100 ml, 0.45 +/- 0.23 mumol/ml, 0.33 +/- 0.15 mg/100 ml and 17.32 +/- 3.85 mg/dl respectively). There were statistically significant differences between ATP (p less than 0.0005) levels and 2,3-DPG and lactate (p less than 0.05). These results force us to study the presence of those changes on peripheral tissues to see whether they play an important role in hypertension complications, especially in respect of the vascular wall.

We evaluate the levels of intra-erythrocytes glucose-6-phosphate-dehydrogenase (G6PD) in a group of 50 patients with essential blood hypertension (EBH) (214.76 +/- 38.31 mU/ml) and in a group of 50 healthy persons (130.11 +/- 49.50 mU/ml) (p minor of 0.0005). At the same time, we have analysed the intraerythrocyte levels of G6PD of 34 healthy sons and daughters of hypertensive parents, detecting high levels in 13 of them. We observed a significant decrease of G6PD after the therapy with nifedipine in 20 hypertensive patients (p less than 0.0005). We conclude that measurement of G6PD enzyme activity can prove to be a good marker of essential hypertension. This change occurs at an early stage and is noticeable in high risk persons with family predisposition. It is possible to modify levels by working on cation changes through membrane cells by calcium-antagonist.

The intestinal absorption of calcium and seric levels of calcifediol and calcitriol were studied in 27 patients with essential blood hypertension (BH) and 20 normal patients in a control group. The hypertensive patients were divided in three groups depending on the plasmatic renin activity (PRA), (high, normal, low). We found a significantly higher rate of intestinal calcium absorption in the group of patients with BH and low PRA than in the control group (22.73 +/- 19.05 and 10.7 +/- 17.13% respectively) (p less than 0.025). The seric levels of calcitriol (1,25 (OH) 2D3) were significantly higher in the group of patients with hypertension and low PRA than in the control group (48.16 +/- 5.25 and 33.13 +/- 2.9 pg/ml respectively) (p less than 0.025). The seric levels of calcifediol (25-OH-D3) were significantly lower in low PRA patients than those patients in the control group (12.37 +/- 2.44 and 21.94 +/- 2.82 ng/ml respectively) (p less than 0.025). We concluded that patients with essential blood hypertension and low PRA have significantly lower calcitriol levels than any other group, probably conditioning the greater intestinal calcium absorption shown in this group.

51 patients with mild essential hypertension (EH) were randomly divided into two groups; the first group (31 patients) was treated with 1 g. of oral calcium during a period of 8 weeks; the second group received a placebo. The treated with calcium showed a significant decrease in blood pressure (BP), the maximal reduction being of 6 mm Hg in the systolic pressure (SBP) and 3 mm in the diastolic (DBP) at the end of the 8th week. We found a substantial positive relationship between the decrease in SBP and seric renin activity (p less than 0.05), as well as a significant negative relationship (p less than 0.05) between DBP and the seric level of parathyroid hormone (PTH). During the administration of the oral calcium supplement, the BP decreased in an inverse proportion to the plasmatic renin and seric level of PTH.
